Cooling Tower is storing excess heat the majority of the time

I have problem for modeling in water side.
Chiller COP is lower than 1 for all the time. Investigating the result, seems that condenser outlet and inlet temperature from chiller is so high (Inlet:53degC - 75degC, Outlet:58degC - 80degC) as shown in histogram below.
Then looking at the err file, found that Plant Loop: SD1 CW LOOP Demand Side is storing excess heat the majority of the time. Plant Loop: SD1 CW LOOP Supply Side is storing excess heat the majority of the time.enter code here
Does anyone know what is causing this warning and how I can fix this problem?

I've had the same problem with my heating loop. I Set the PlantLoop: Plant Loop Demand Calculation Scheme temperature setpoint to my loop design temperature and it get solve. I mean I'd set this setpoint at 60 deg and my boiler had been set on 82 deg. so I changed PlantLoop: Plant Loop Demand Calculation Scheme setpoint to 82 deg and it get solve.
